Remove the 3-argument constructor for FixMessage and adjust all affected
usages. Both the fix message and replacement are now required. This
found a few bugs in the testing.
Also remove the unused must in the toDescription lambda and
shouldContain in searchReplacments. We can add them back in in case we
want to add negative testing for fix-its in the future.
