Flatbush Avenue District Management Association, Inc. (d/b/a Church-Flatbush Community Alliance), Queens Lighting Collective, the Local Center, and the NYC Dept. of Small Business Services desire to use, display and publicize photographs submitted for an installation on E. 18th Street. By submitting this form, I hereby grant the abovementioned parties the rights to use my photograph, image and likeness in any medium for said installation, including in and on brochures and other print publications, electronic and optical media, display and any digital transmission or delivery methods, on any platform and for promotion of the installation.
Flatbush Avenue District Management Association, Inc. (d/b/a Church-Flatbush Community Alliance), Queens Lighting Collective, the Local Center, and the NYC Dept. of Small Business Services shall be the exclusive owner of all rights in all materials that incorporate your photograph, image and likeness. The individual submitting this form acknowledges and agrees that there is no right to review or approve any such materials before they are used by these parties. The individual agrees not to submit any images containing nudity, offensive content or inappropriate language. The individual holds these parties harmless and acknowledges that the parties have no liability for any editing or alteration of images submitted via this form. Any credit or other acknowledgment of me, if any, shall be determined by these
parties in their sole discretion.
