Add automatic purpose notes to each new draft revision

We should save the metadata of why each draft revision was created. For example, a draft revision created from an rsync upload should have a note to that effect. A draft revision created from a file deletion should note that it was created due to that, and so on. These should then be displayed on the revision history page.
