This form is to indicate that you are FRIENDS or acquainted with a PNM outside of recruitment and thus cannot interview them or vouch for them in deliberations. Rule of thumb: if you would feel it be awkward if you had to interview this person, put their name down! This form is MANDATORY for anyone conducting interviews. If you have more than one COI fill this out multiple times.
