SLAQC Meetup Feedback & Incident Report Form
SLAQC meetups exist to create a supportive space for all queer people to enjoy climbing. Further, we aim for our meetups to embody our norms and aligned expectations for participants. We welcome your feedback below on how we are meeting mission. You may leave feedback anonymously or provide contact information for us to follow up with you on your feedback. This form also serves to collect information on incidents that occur specifically at SLAQC events (versus outside of our sanctioned meetups but at our partner gyms). Incidents may fall into the categories of climbing incidents (injuries etc.) or relational ones (conflicts, micro-aggressions, etc). While these incidents have been extremely rare in SLAQC's history, this form serves to ensure SLAQC leadership is aware of and able to respond to incidents if/when they occur. Leadership will review meetup feedback on a monthly basis and incident reports ASAP as they come in.

SLAQC Guidebook Lending Library Request Form
Please fill out the form below if you would like to borrow a guidebook from SLAQC's lending library. These are intended to be a shared resource for our community. Please take good care of our books! - Maximum loan period is 14 days (i.e., you must return the book within 14 days of borrowing it). Repeated late returns will preclude you from future participation in this program. - A deposit is not required. If a book is lost or damaged we hope you will help us replace it. - Pick up and drop off is in South Salt Lake (near the Front SoMa) OR at a meetup If you would like to donate a book to the library, send us a DM on instagram or facebook!
