INSTRUCTIONS: Please rate your student-learner’s attitude or performance by placing a letter grade, A-E, on the line provided. An “A” is the highest rating. The following can be used as a guide for evaluating each category: (A) Exceptional attitude and performance (B) Complete satisfaction with level of performance and attitude (C) Usually satisfied, though occasionally makes small errors (D) Acceptable; improvement would have to show for employee to be considered a fully competent employee (E) Unacceptable; must improve for student to maintain position without probation or other measures Thank you for taking the time to fill out these questions.
