For more than two years, the Global Coalition on Aging has been leading the engAGE With Heart program in #Baltimore. We've learned that improving health in Baltimore is about more than improving access to health services. It's also about engaging with the community within our trust-based model to identify and tackle social barriers. We are so proud of the progress we're making in Baltimore and our new partnership with Uber to bring better and more timely care to those who need it most. ### When we hear that a community member has skipped a doctor’s appointment because they couldn’t find a way to get there, we’re reminded just how much health depends on access. At engAGE With Heart, we work every day in East and West Baltimore to confront the realities that make heart health difficult to maintain. Our program, built for and with the local community, brings together families, churches, senior centers, and trusted local partners to close these gaps. Through accessible education, healthy food resources, and health screenings, our team is meeting people where they are, connecting residents to the support they need to live longer, healthier lives. That’s why we’re proud to announce a new partnership with Uber. Through this collaboration, our Community Health Ambassadors (CHAs) can arrange reliable rides so community members can get to medical appointments, pick up medications, access healthy food, and reach the services that support their heart health and overall well-being. For so many in the neighborhoods we serve, one ride can change the trajectory of their care. It can mean catching a diagnosis early instead of too late and staying connected rather than isolated. When transportation is no longer a barrier, our community members can stay present with their care and build towards better heart health, greater independence, and a stronger community. This is the work that makes a difference. Learn more about our partnership with Uber here: https://bit.ly/49Xp4m6
