LIFTING PLAN execution/proceedure.

Definition
Lifting is a process for Lifting or lowering the different type of load with the help of lifting
appliances or manually.

To conduct lifting operation,,,


1. A lifting plan and risk assessment should be available with the permit. and ensure all
necessary controls are in place. Like area Barrigation, signboards.
2. Check the soil condition and then Place the Crane Close to the load as written in lifting
plan,and Verify THE LOAD CHART,WIND SPEED and ensure the safe distance between
load and crane.
3. Inspect TPI of crane and and lifting accessories.
4. Barricade lifting area properly and Put Sign Boards.
5. Fix the hook to the load using the correct method of rigging, according to the shape of
the load.
6. Ensure that a single banks man is communicating with the Operator.
7. Gradually lift load till 6 inches and hold for a While to ensure the stability of the load and
crane. If Stable execute the lifting operation.
8. Ensure that the hook is Disengaged after the load is securely placed.
9. Unauthorized person and vehicle not allowed in lifting areas.
10. Continue supervision until work finishes.

MSAD:minimum safe approach distance.


1. For 132kv portal design and steel tower power-line minimum safe approach distance
should be 3.5m.
2. For 66kv and 33kv power line MSAD should be 3m.

What are the hazards of lifting?

HAZARDS CONTROLS

Lifting area.loose soil. Barricade area, check soil condition before placing crane.

Vehicle movement/ swinging Maintain safe distance. Never stand behind and in front of
vehicle.

Hydraulic system. Ensure there is no leak with hydraulic pump, assembly, and
pipes.

Suspended load. Load should be stable. never stand under suspended load.
Wrong belts shekels & Use calibrated tools only.
defective tools.

Sharp edges. Remove sharp edges putting rubber or cloths.

Heavy wind speed. Stop lifting if wind speed more than 16m/s.boom should be
lower condition.

Un Competency. Provide competent workers/crew.

Miss communication.

and dimension of load, when it is placed.

Dimension of load on trailer?


When load is Placed ensure dimension,like
1. Distance from head board = 1 meter.
2. Total height of load with trailer - 4.5 upto 4.8 meters.
3. Total load width on both side of Trailer= 3.5 meter.
4. extension of load from rare excel = 1.5 meters.
after load is Placed use Proper belt and Chain to ensure Stability of load and movement is
avoided.

what is Lifting test?


Lifting test means, to lift a load slowly till 6 inches to check stability of load.

Lifting tool/accessories
1) Lifting appliances [[crane, telehandler, forklift, sideboom]]
2) Chain sling,Web slings,Chain lift,Chain block.
3) D Shekels, Bow Shekels.
4) Clumps,Hook,Jib.
5) Lifting beams.

Lifting types/categories?

Lift categorization segregates types of lifting and hoisting operations based on the risk
level.

There are two main types in pdo

Routine lifting/generic
Non Routine lifting

Non Routine lifting has further two types Simple and complex lifting.

ROUTINE LIFTING/GENERIC LIFTING?

Approved by AP.
Routine lifting is repeated on a regular basis in which we lift load less than 5-ton using a single
crane, forklift, and other equipment using basic rigging methods in the non-sensitive areas.

Ensure lift gross weight is less than 5 tons.

CONTROL:for genric lifting.

A lifting plan and risk assessment should be available with the permit.
Pre-use inspection, barricade lifting area.
Conduct TBT and 10 questions for safe lifting.

Required crew; permit holder, lifting supervisor, operators, Signaler, Slinger, flagmen.

NON ROUTINE LIFTING?

There are two main types in non-routine lifting.


1 Non Routine Simple lifting.
2 Non Routine Complex Lifting.

1 Non Routine Simple lifting.

Approved by company LIfting engineer.


Non-routine simple lifting is used to lift load less than 20 tons using single crane and correct
method of rigging. ensure lift gross weight is less than 20 tons. Gross weight is more than 50%
but less than 75% of the crane capacity. Ensure CG of the load. ensure lifting point is directly
above the load.

When we use single crane for lifting operation?

Use single crane for lifting if working within narrow, restricted, live process area, in night
operation, during blind lift, or if lifting required in confined space, trench, or excavation.

CONTROL:
A lifting plan and specific risk assessment should be available with the permit.
Pre-use inspection, barricade lifting area.
Conduct TBT and 10 questions for safe lifting.

Required crew; permit holder, lifting supervisor, operators, Signaler, Slinger, flagmen.

2 non routine complex lifting.

Approved by company LIfting engineer.


is used to lift load more than 20 tons and which is heavier over 75% capacity of lifting
equipment, using one or more lifting appliances within difficult or restricted areas.

Like as
● Lifting over hydrocarbon containing equipment,or lift over live plant.
● Lifting near OHL,,
● transfer load from one lifting appliance to another.

CONTROL:

lifting plan,Risk assessment and checklist should be available with the permit.
Formal written work package and Use a S M E consultation.
Toolbox talk.10 questions for safe lifting.
Required crew; permit holder, lifting supervisor, operators, Signaler, Slinger.
What is tailing lift?
A lifting type where load is transferred from one lifting equipment to another.

Exp: placing an horizontal electric pole into vertical position.

10 Questions for a Safe Lift

● Is everyone aware and fully understand the lifting and hoisting procedures?
● Has everyone attended the toolbox talk?
● Has a pre-use inspection of the Lifting Equipment been carried out and are the Lifting
Accessories marked with:
Safe Working Load
A unique identification number
A valid certification date.

● Are all safety devices working?


● Does everyone know the Person-in-Charge of the lift?

● Is everyone competent and aware of his tasks?


● Is there current Lift Plan and JHA available and does everybody understand the job and
precautions?

● Does everyone know the environmental limits?


● Is the lift area controlled and is everyone clear if the load falls or swings?
● Are signalling and communication methods agreed and clear to you?

Lifting Specifications 2273

Definition
Lifting is a process for Lifting or lowering the different type of load with the help of machinery or
manually.

Where sp2273 used?


Sp 2273 shall be applied all over the pdo for all Lifting operations and shall be followed by all
companies and contractors

Purpose of sp 2273?
The purpose of sp 2273 is to implement and control all Lifting activities and to define individual
responsibilities of Lifting work party.

Lifting equipment or appliances?


Lifting equipment defined as the machinery that is used to lift or lowering the load including its
attachments. For example mobile crane, crawl crane, tower crane, forklift, telehandler etc.

Lifting accessories or attachments?
Any equipment or components attached to the Lifting machine in between load and hook
as web sling, chain ⛓ sling, belts, d shackle, bow shackle etc.

What is load?
Load can be any material in different weights, sizes and shape. For example pipe, drum, Tanks
and other equipment.

Exclusive zone in Lifting?


It is barricaded area with tape or fence to avoid access for unauthorized person and vehicle in
lifting areas.

Competency/ Competent person?


A person having valid documentation knowledge, skills,practices, experience of Lifting
operation.

Management of Lifting ?
A safe system of work shall be follow for each crane operation even it is single or group Lifting.

People involved in lifting?


1. Asset Manager
2. Lifting SME
3. Contractor ,Contract holder
4. Vehicle Marshall
5. Appointed person
6. Permit holder,Lifting Supervisor,LEC.
7. Equipment operator, Signaler, Slinger.

Asset Manager?
Is responsible for ensuring the activities that he is Controling are managed safely according to
the requirement of sp 2273.

SME LIFTING SUBJECT MATTER EXPERTS?


is the technical authority for providing lifting consultation, also provide Verification methodology
and procedures for complex lifting .

Contract holder?
Is responsible for communicating the requirements and specifications 2273 to the contractor.

Contractor?
Contractor is responsible for several things,
1. All lifting risks have assessed and control have been implemented.
2. Lifting operation is planned and carrying out in a safe manner.
3. All relevant information have been provided to the lifting team. And team will conduct
work by following instructions.
4. Reporting system for removing any defected machines.

Vehicle Marshall?
Is a person authorized to control and manage machinery movement and parking.
He is also responsible to perform safety checks.

AP?
Is an appointed person acts on the behalf of organization and is responsible to control overall
lifting operation.
He also responsible for
1. Planning,developing and approving lifting plan.
2. All necessary controls have been implemented.
3. Select certified, suitable and maintained crane.
4. Seek additional support for lifting if required.
5. Conduct visits and examine worksite, and ensure all risks, incidents are reported and
recorded.
6. Provide proper handover to lifting supervisor.

Lifting Supervisor?
Is and authorized person with valid documentation to supervise and control lifting operation.
He should has
1. Proper knowledge training and experience and fully understand lift plan and risk
assessment.
2. Ensure site is safe for lifting and workers are competent.
3. Conduct toolbox and 10 questions for lifting.
4. Ensure validity of equipments.
5. Responsible to make changes and stopping the work in case of unsafe situation.
6. Supervise lifting activity until end.
LEC?
A person to control and maintain the loft, he will
1. issue and accept all accessories when return.
2. Conduct pre and post checks.
3. Maintain loft register,ensure accessories are certified and color coded.
Opretor ?
Operator is one of the most important person of lifting team. He should be able to read and
understand load charts.
Operator should
1. Pre check lifting machine and operate specific machine he is authorized for.
2. Seek permission before Operator and follow lift plan.
3. Ensure slingers and matts are in place and
4. Stop work if unsafe and never leave machine unattended.
5. Always park machine in parking area.

Signaler/banksman?
Signaler is responsible to control machine movement by giving signals to Operator.
He should have proper training and valid documentation. He should be able to judge weight,
distance height etc. He can play role of lifting supervisor in routine lifting.

Slinger/rigger?
Is responsible to attach and de attach the lifting accessories. He should be trained and able to
select and handle lifting accessories. He should ensure accessories are certified,valid, color
coded and in good condition.

Special requirements for rigging?


● Never use Unf thread eye bolt and stainless steel shackle.
● Never use Shur lock hook for pipes and plate.

Operation manual?
Operation manual contains information for an equipment to be used on different operations.

Test certificate?tpi
A Certificate is issued by third party that contains information about lifting equipment.certificate
indicates that equipment is fit or not.

Daily inspection report?


Is conducted by operator to ensure that equipment is fit and fully functional.

Maintenance requirements?
All lifting equipment shall be maintained according to the direction of manufacturer or asset
owner.
Sop's for developing lifting plan.?
1. Site survey, risk assessment,
2. Selection of crane,
3. Lift plan, TBT and communication,
4. Operation area,
5. Evacuation and Emergency plan,
6. Reaction to change lift plan.
A.p will conduct Worksite examination and get detail for ground conditions weight size
and route and ensure area is Safe and barricaded. identify risks related to work and set
control. Select suitable and certified crane. A lift plan ptw and risk assessment is
compulsory for lifting. Arrange means for communication in between operator and
banksman. Conduct TBT with crew, explain emergency plan and evacuation route.when
actual information doesn't meet with lift plan stop work until approval.

Safe slope distance?


The pressure of outrigger mat can pass to the trench at an angle of 45°.

So The distance in between mat and bank of trench should be greater than doubledepth of
trench.

D+d >2H(depth) A.P will seek SME advice concerning this if has any doubt.

Assembly and de-assembly of large cranes?


Assembly and disassembly is a high risk job, there are many hazards. work at height certificate
and continuous supervisionis required. Manufacturers instructions shall be followed.

Use of taglines?
Taglines are used to pull, push or swing the suspended load. Operator shall approve how many
taglines are required.
● Never wrap taglines around body.
● Taglines should be long enough to maintain safe distance.
MEWP mobile elevator work plate form?
1. Mobile elevator work plate form used to lift worker never ever use for any other thing.
2. check for wether conditions and follow lift plan.
3. provide competent workers and ensure visibility and proper communication means.
4. Keep the body and and necessary tools inside before raising or lowering and don't
overload elevator.
5. Wear fall protection and hold with both hands and prepare for emergency.


6. Never support elevator with structure.
7. In case of emergency stop work and re-assess.
8. Stow(boom fully down ) and shutt of all power before leaving.
Non integrated work platform?
Non integrated working platforms attached with lifting appliances are not permitted.

Difference between integrated and Non integrated work?


Non integrated work Platform have no controls in the platform to lift or to move the truck;

i.e. all forklift


working platform movements are controlled by the operator.

Integrated platforms have controls in the platform to lift or to move the truck. Worker in the
platform can control trucks by himself.

Lifting operations near OHL ?


If lift work is within 15m area of OHL than safety barriers shall be erected.
Maintain minimum safe distance by 6m.
Supervisor shall ensure crane does not operate within 15 meters, and measure boom length
horizontally for power lines on steel towers.
MSAD: minimum safe approach distance shall be use for safety clearance.
For portal design and steel tower lines of 132kv distance is 3.5m .
For portal design 66kv and 33kv distance is 3.0m.

If work is directly under OHL than Line should be made dead and electrical representative shall
ensure minimum safety clearance 3.5m distance vertically.
Night lift operation ?
All night lift operations classified as non Routine lifting. It is approved by lifting engeenier.
● Make arrangements for lighting and backup lights.
● Escape route should be illuminated.
● Make arrangements for signaling and communication.
● Define assembly point

Live plant/equipment?
Live plants are equipment that contains hydrocarbons, pressure and energy sources. like
pipelines, vessels, tanks and power cables etc.

Lift over live plants?


Lifting over live plant is complex lifting, it has many hazards and risks.
That's why,,
Lifting over live plant is prohibited,but if we have no other option then ensure all risks has been
mitigated or reduced to ALARP.
All lifting over plant shall have approved lift plan for controling fire, explosion, electrocution
etc.and exclusive zone shall be determined.

Handling of tubular?
Handling Of cylindrical materials like pipe contains many hazard that is why it should be
planned. Ptw,lift plan and risk assessment should be followed.
Work party is responsible for selecting right means of lifting, handling and transportation of
tubular.
There are some best practices to handle tubular.
● Using attachments

● Using fork
● Using hook
attachments Shall be OEM and TPI approved and suitable.
Tubular should be secured during forklift movements.
Using hook slings should be placed at equal distance and double wrap around tubular.

Drilling And rig move activities?


Approved lift plan should be available for All lifting related to drilling and rig move.
Company will approve lift plan for one year.

Application and step out?


Lifting Specifications 2273 is applicable for all type of lift operations and should be followed by
company and contracts.

Contract Lifting operation?


Before Contract company shall confirm that contractor has the capability to perform work.
Weather conditions during lifting?
During the planning of lift plan Weather conditions shall be considered and add in risk
assessment an continuously monitor during lift operation.
There are some Weather effects?
● Rain
● Wind
● Poor visibility.
A p shall ensure precautions are taken to avoid danger if crane or load can be effect by Rain or
wind.
If there is Poor visibility by fog or sand ensure proper means of communication.
Stop work if wind speed,rain or Poor visibility exceed safe limits.

Lift Plan specication

A.p (appointed person) shall create lift plan.


A lifting plan is a written procedure that provides complete instructions for lifting different types
of materials or objects.
All lift operations shall have lift plan,ptw and risk assessment.

Lift Plan number,title,description,scope,location,date and revision number should be available.

Method of communication and Hands-Free Lifting to be used (e.g. Tag Line, push pull poles)

Lift category ,names and Signatures of persons who provided approval.

Details of load including dimensions weight and Centre of Gravity.

Details of the Lifting Appliance(s),assessries and SWL should be available.

Applied ground pressure and Capacity of ground.

Number of workers and their roles.

Method statement including step by step procedure).


Sketches and drawings.

Rigging arrangement (lifting accessories) and calculations.

Task risk assessment and hazard analysis should be done.
