At some point in the game, when i use the M203 launcher, the grenade won’t detonate thus wasting my action points and doing no damage

I found a bug that could cause that, but it only happens if you have a fast GPU and have disabled VSync. As a temporary workaround you could try and enable VSync and see if you still have the problem
